Waterpipe (shisha, hookah): nicotine content

A waterpipe session is not comparable to a cigarette in either duration or smoke volume, and the water does not remove nicotine. The pack tells you the weight of the mixture and nothing about the dose.

This is a reference page about what is printed on packs and what EU law caps. It is not medical advice, not a diagnosis and not a recommendation to use any product here. Nicotine is addictive and every product on this page can sustain that dependence. If you use nicotine and want to stop, a pharmacist, a doctor or your national cessation service will help you more than a table will.

A session is a very large amount of smoke

A cigarette is finished in five to ten minutes and delivers of the order of ten puffs. A waterpipe session commonly runs twenty to eighty minutes with a puff count in the hundreds, and each puff draws a much larger volume through the hose. The World Health Organization study group on tobacco product regulation has put it in the terms most often quoted since: a waterpipe user in a single session may inhale a volume of smoke comparable to that inhaled by a cigarette smoker consuming a hundred cigarettes or more.

Smoke volume is not the same as nicotine dose, and it should not be read as one. But measured nicotine uptake over a session sits in the range of several cigarettes rather than a fraction of one, which is enough to make the common description of shisha as an occasional and mild activity misleading.

The water cools the smoke; it does not clean it

Passing smoke through water lowers its temperature, which makes a large puff comfortable to take. It does not meaningfully remove nicotine, and it does not remove carbon monoxide at all. The smoothness that the water produces is the mechanism by which a waterpipe user takes in far more smoke than they would from a cigarette, so the filtration belief inverts the actual effect.

Carbon monoxide is the acute risk

Most waterpipes are heated with charcoal sitting on top of the bowl, and the charcoal is a large and continuous source of carbon monoxide independent of the tobacco. Acute carbon monoxide poisoning after waterpipe sessions, particularly indoors and in poorly ventilated rooms, is a recognised presentation in emergency medicine. Headache, dizziness, nausea and fainting during or after a session are not a normal part of the experience and are a reason to get outside and seek help.

Herbal and tobacco-free shisha

Nicotine-free molasses mixtures exist and are sold as a tobacco-free alternative. They contain no nicotine, so they cannot maintain dependence, but they are still burned material inhaled through the same apparatus with the same charcoal, so the carbon monoxide exposure and much of the particulate exposure remain. Tobacco-free is a statement about nicotine, not about smoke.

Against a cigarette

The only figure that lets any of these formats be compared is what a smoker actually absorbs: roughly 1 to 1.5 mg of nicotine per cigarette, or something in the region of 20 to 30 mg across a twenty-a-day habit. That is a measured absorbed dose, and it is not the same kind of number as the one on most packs.

This is burned or heated tobacco reaching the lung, so the speed is in the cigarette range even where the total nicotine is not. Comparisons on total alone will understate how reinforcing it is.

A content figure and an absorbed figure are different quantities. This table puts both on one page precisely so that the difference is visible.

Quitting from here

Waterpipe use is often social and occasional rather than daily, so the useful lever is usually the situation rather than a replacement dose. Where use is daily, the dependence is nicotine dependence like any other and the licensed products apply.

The pattern with the most evidence behind it is the same whichever product you are stopping. A long-acting form holds the baseline so that withdrawal is not generating cravings all day, and a fast-acting form is there for the moments a craving still arrives. Cochrane reviews of nicotine replacement therapy find that using it improves the chance of stopping compared with no medication, and that combining a patch with a fast-acting form does better than either on its own. Behavioural support on top of medication does better again.

Two things are worth knowing before you start. Under-dosing is the usual failure: people choose the low strength, use it less often than the label says, and conclude that replacement does not work for them. And prescription medicines exist for tobacco dependence as well; those are a conversation with a clinician rather than a shelf decision.

National services are free and staffed by people who do this all day. In Estonia the National Institute for Health Development runs cessation counselling and publishes the current clinic and helpline list. In Finland the Stumppi service run by Filha covers the same ground. In Sweden it is Sluta-Roka-Linjen. Your pharmacy will know the local one.

Frequently asked questions

Does the water filter out the nicotine?

No. The water cools the smoke and makes larger puffs comfortable. Nicotine passes through, and carbon monoxide from the charcoal passes through untouched.

How many cigarettes is one shisha session?

In smoke volume, WHO puts a session at a hundred cigarettes or more. In nicotine, the honest answer is several cigarettes rather than a hundred. The two measures are not interchangeable.

Is tobacco-free shisha safe?

It contains no nicotine, so it does not maintain dependence. It still produces smoke and the charcoal still produces carbon monoxide.
